Service Description

1. Initial Assessment: In person home visit for Home Care OR On-line visit for holistic services A health professional uses a systematic, dynamic way to collect and analyze data about a client. Assessment includes not only physiological data, but also psychological, sociocultural, spiritual, economic, and lifestyle factors as well. Please share any information in relation to your condition. All collected information is kept confidently for the care plan development only. Many nursing tasks can be delegated to unregulated healthcare professionals and supervised by nursing staff. However, there are tasks that only a nurse can perform. Nursing professional determines that during the home assessment. 2. Diagnosis The nursing diagnosis is the nurse’s clinical judgment about the client’s response to actual or potential health conditions or needs. A medical diagnosis deals with disease or medical condition. A nursing diagnosis deals with human response to actual or potential health problems and life processes. For example, the diagnosis reflects not only that you are in pain, but that the pain has caused other problems such as anxiety, poor nutrition, and conflict within the family, or has the potential to cause complications. 3. Planning - done remotely Based on the assessment psychosomatic analysis and diagnosis, we formulate and share the goals of care. We set measurable and achievable short- and long-range goals for you. This work can bring emotions and even physical reactions. We may use emotional freedom technique to move through. We may need a few planning sessions. 4. Implementation Your care is implemented according to the care plan. You will have a clear step by step implementation plan. It is highly important to execute on 3 dimensions to sustain the change. Caregivers are training and supervised directly and indirectly during this stage. The frequency of nursing visits are based on your condition and care needs. 5.Evaluation Both your status and the effectiveness of the care plan must be continuously evaluated, and the care plan modified as needed. Upon discharge from Nursing or Caregiver assistance, we’ll meet to conclude the results. The home care can be short term or convalescent. Convalescent care provides a home-like environment during post-surgery recovery, injury recovery, and can even be used as a transitional form of care following stroke or a lengthy illness.

The session is considered completed once Client and Contractor are connected online at the scheduled time; ii. The appointment time may be scheduled if requested 72+ hours prior to the scheduled time considering the provider availability. iii. A group session attendance may never be rescheduled, see Terms for details.
